This information will be used to ensure the safety of your camper. Please fill it out to the best of your ability so we have the necessary information about your camper.

Authorized pick-ups:

Campers will only be released to authorized pick-ups. All authorized pick-ups must bring government issued identification and present it at the time of pick-up.

Allergies:

List all allergies (even if not severe) and approved medications to treat allergies. Due to state regulations Colorado Youth Outdoors cannot hold onto and/or administer any medication.
